In addition to the completed form please provide the following:
A. The value of any real estate you own or are buying – you can go to www.zillow.com for a free approximate valuation of your home (this valuation is for comparison purposes only and may not be accepted by the court);
B. Proof of the last six months income: from ALL sources (include part-time and temporary incomes). If you are married we will need your spouse’s paystubs (even if he/she is not filing with you). If you have a fixed income, such as social security, you may bring an award letter as proof of income. If you are self-employed, we will need your last year’s tax returns and a profit and loss for the last six months.
C. Your most recent tax return: If married and you file taxes separately, please bring your spouse's tax returns as well. WARNING: to be eligible to file bankruptcy you MUST be current with all personal income taxes (i.e. - all tax returns that are due must be FILED). Our office may not be able to advise you properly if you are delinquent with your tax filings. If you owe taxes it will be important to know the following:
1) what years you owe for and how much is owed for each year (list each year separately)
2) when the returns were actually filed (if filed late)
3) Whether you filed for an extension for the year that is owed
4) Whether you have submitted an offer in compromise to the IRS or State
5) If the IRS or state has filed a tax lien or levy
D. Make sure that page four (4) of the consultation form is completely filled out. List all your creditors including their names and balances. If you have a credit report, we will still need you to summarize the creditors on page four. You can get a free credit report at annualcreditreport.com by clicking the below tab
